Diabetes Care

Our Diabetes Self-Management Education (DSME) program is certified by the American Diabetes Association.  Undiagnosed or untreated diabetes can cause a wide range of health complications and concerns.  DSME provides people living with diabetes the tools to lead as healthy and normal lifestyle as possible.

CHI St. Gabriel’s Health’s diabetes educators work with you and your provider(s) to develop an individualized treatment plan that will encourage you to make the lifestyle changes related to your diabetes care to improve your overall health and reduce your risk of developing complications associated with the disease.  This individualized treatment plan will help you “live health with diabetes,” empowering you to take back control of your disease.

Diabetes care education includes information on the following:

  • The diabetes disease process
  • Nutrition management
  • Meal planning
  • Blood glucose monitoring including the option of continuous glucose monitoring
  • Diabetes medications
  • Family support
  • Exercise and diabetes
  • Preventing, delaying, detecting and treating complications
  • Goal setting and problem solving

Diabetes Prevention

The diabetes education center offers “Health Begins in the Kitchen,” a comprehensive program to help individuals lower blood sugar, cholesterol, blood pressure, and lose weight.  The class includes education, discussion, taste-testing, and food preparation.  Participants also receive numerous healthy recipes.

The program is offered several times throughout the year.  Each class series consists of four two-hour sessions.  The cost is $45 for all four sessions.

Dietician Services at St. Gabriel’s Hospital

At St. Gabriel’s Hospital, dietician services are focused on assuring patients are consuming an adequate amount of their diet recommendations to promote healing for various conditions.   The hospital and cafeteria menu promotes “in-season” foods that increase the nutrient density of the overall meal while meeting dietary requirements.

Dietician Services at Family Medical Center

At Family Medical Center, the dietician will work with patients to meet their weight-loss goals.  Patients receive guidance to identify and adjust areas of their current lifestyle that will aid in weight loss and establish healthy habits.

The dietician will also work with patients who have elevated cholesterol and blood pressure. Care is personalized to the needs of each patient.

Certified in adolescent weight management, our dietician helps adolescents and families establish healthy lifestyle habits at a young age and achieve a healthy growth curve.
